You are being asked to participate in a research study and complete this survey because you are a registered nurse currently contracted, previously contracted, or have interest in being contracted with Trusted Health. The purpose of this study is to describe the health and healthy lifestyle behaviors of nurses during the Covid-19 pandemic and compare that with the health and healthy lifestyles of nurses in various roles. This survey, which will take approximately 15 minutes to complete, asks questions about yourself and your overall health and wellbeing. After submitting the survey, you will have the option of entering a raffle to receive a $100 Amazon Gift Card. To maintain anonymity, the raffle link will direct you to a separate page that will have no connection to your survey answers. 

The research has been determined exempt from IRB approval and your survey answers will be anonymous and voluntary. Because we are using the internet, there is a chance that someone could access your online responses without permission. In some cases, this information could be used to identify you. All data collected will be presented in aggregate form. No personal information will be shared. Completion of the on-line survey will be considered your consent to participate. All answers, including the demographics, are optional. We appreciate it if you can complete the entire survey. There are no right or wrong answers.
