Study Title: The State of Health and Healthy Lifestyle Behaviors in Big 10 Health Sciences Faculty and Students

You are being asked to participate in a research study and complete this survey because you are a faculty or student at one of the participating universities’ (Indiana University, Michigan State University, Ohio State University, Pennsylvania State University, Pennsylvania State University - University Park, Penn State Health Milton S. Hershey Medical Center, Purdue University, Rutgers University, University of Illinois at Chicago, University of Iowa, University of Maryland, University of Michigan, University of Wisconsin – Madison, University of Wisconsin – Milwaukee).  The purpose of this study is to assess the health and healthy lifestyle behaviors of Big 10 health sciences faculty and students. The study will also compare healthy lifestyle behaviors and the health of Big 10 faculty and students across health sciences’ professions.  This survey, which will take approximately 10 minutes to complete, asks questions about yourself and your current knowledge and practice around healthy lifestyle behaviors.

Your survey answers will be anonymous and voluntary. Because we are using the internet, there is a chance that someone could access your online responses without permission. In some cases, this information could be used to identify you. All data collected will be presented in aggregate form. No personal information will be shared. Completion of the on-line survey will be considered your consent to participate. All answers, including the demographics, are optional. We appreciate it if you can complete the entire survey. There are no right or wrong answers. You can skip any questions you do not wish to answer.

You will not receive any direct compensation for your time.
