You may only submit one Transfer Credit Evaluation Request for all of your math coursework.
If you have completed more than one math course, you will need to submit materials for all of your courses at once on this online form. Students must have all math courses evaluated. It is not possible to evaluate only some of the math courses completed. Students should also submit materials for courses they are currently enrolled in; completion of the course(s) is not necessary.

 

Note: Statistics course are not evaluated by the Department of Mathematics. Students should contact the Statistics Department to have their statistics courses evaluated.

Please have a copy of your Transfer Credit Report available as you will be asked to provide information from this document. This form cannot be saved and continued later. The process will take between 10-30 minutes depending on the number of courses you have completed.
