Please use this form to schedule surgical, anesthetic and other related technical services for ULAR large animals and USDA covered species. 

• Request must be submitted at least five (5) business days prior to the requested procedure date.
• All required fields must be completed or the form will not be accepted. 
• All requests must have IACUC approval.
