Permission to Reactivate Enrollment Eligibility

Enrollment eligibility is removed from students who do not enroll for a full semester. Fill out this form to request your reactivation of enrollment eligibility so that you may register for courses.

Note that students who have not enrolled for two calendar years must have their reactivation approved by their Graduate Studies Committee chair.
