USAC Staff in the Spotlight Nomination Form

Please use this form to nominate an employee for the USAC “Staff in the Spotlight” recognition program.

Nominations are due to the USAC Staff Affairs Subcommittee by the 1st Friday of each month.

Nominations will be considered for the three months following receipt of the nomination.

Purpose: Employee spotlight is designed to recognize staff of The Ohio State University who exemplifies what it means to be a Buckeye.

Recipients will be selected on the following criteria:

• Strives to bring out The Ohio State University best practices in others.
• Demonstrates Ohio State's Shared Values
• Creates a positive impact on others in The Ohio State University Community.

Eligibility: All part-time and full-time staff in a regular non-faculty position may be nominated

Reason for Recognition: The following is provided to help describe key qualities, traits, or characteristics of the nominee.

Nominations and nominators may be shared with the nominee and the USAC Newsletter, USAC website, and/or social media
